Patient Information

Upon scheduling your first appointment at Spinal Health Chiropractic Center, patients will fill out a “New Patient Form” covering the patient’s medical history, current health condition, the patient’s insurance information and any other needs of the patient.

Once we have obtained all of the necessary information, we will begin the appointment. If the patient prefers, he or she can print and fill out the form out at home or fill out the form at our office the day of the appointment.

Patients will be able to ask questions about the process and inquire about any other concerns they might have. We will be more than happy to go over everything in detail. Patients can also utilize our website to read over the information on various treatment methods, the types of treatment we offer, and information about our team and chiropractic center.
